What are Medicare-certified home health services?

Short-term home health care services are prescribed by a physician to help with rehabilitation after an illness, injury, hospital stay, or surgery, or to help manage a chronic medical condition with the goal of preventing an unnecessary hospitalization. This care is provided through one-hour home visits from specialized clinicians and continues until recovery goals have been met. Episodes of care typically last between 30 and 60 days, but can be renewed if medically necessary. Home health care visits are generally covered 100 percent by Medicare through the Medicare home health benefit or by private insurance.

Your RN clinical manager oversees and manages your care plan and caregivers to ensure your care needs are met, and also communicates with your doctor as needed. Your client services manager helps coordinate your care schedule and serves as a primary contact for any questions you may have.
